Abebooks, Amazon and Ebay return nothing.

It's on Amazon in the original Spanish if you can handle that.  Theoretically one could type the Spanish up and let Google translate it.  Or maybe there is a PDF floating around, but I did not see one.

Thank you, Davis! Unfortunately, I don’t read Spanish. And my Internet search, like yours, has been unsuccessful. Hopefully, I can locate a copy somewhere.
