consecration of children to Our Lady

I had a lovely prayer to consecrate children to Mary, under the title of (I believe) the Immaculate Conception.  It was something you could pray daily if you wished.  Of course, I have lost that little slip of paper, and my internet searches have not brought up the one I remember.  Any suggestions of ones you use often for the children?
